As mentions before in this forum, the pi:s are thoroughly tested, failure to get them working are almost every time mistakes behind the keyboard, and I believe it apply this time also, have not figured out what I done wrong yet though.Your situation is very unusual, thousands of downloads a day of PiOS with no problems similar to what you are seeing reported. I wonder if there is a HW fault with the SD card slot, becaue it certainly does sound a bit like a HW problem.
And I have to say that I was a little frustrated on not finding anyone who had my problem, fixed it with a smooth motion and then posted the solution all over the internet.
I never had this much struggle with a Pi before since I started to use them a long time ago. And I don't consider myself a stranger to fiddle with the strangeness of Linux-distributions since I'm a avid Gentoo-user for the last 15 years.
I believe the card-slot is working since I can mount a sd-card there after booting from my USB. Can be that the 6 different sd-cards I tried with are broken all of them. I'll try it with a couple of new ones in a week.
